We need some more information from you in order to fully organize your employment. This is because your employer may use a wage cost benefit or no-risk policy in the event of illness for employees who meet certain conditions

The questionnaire has four questions about the situation prior to your employment at your current employer. We would like to ask you to fill in these questions, even if you can fill in ‘no’ everywhere. Participation is easy and fast.
